Group H is available to address rare and/or exceptional situations or for individuals with extremely complex needs and/or extraordinary circumstances. Group H is a designation made by prior approval that is in addition to your NM DDW Group (A-G). The Group H approval allows for additional service types and amounts, if you have needs that go above and beyond the service package corresponding to your NM DDW group. If you think that this might apply to your circumstances, check with your support team, and your case manager can fill out the necessary application forms.
Temporary Group H Approved Through DDSD Regional Office
If you have an immediate need for short-term additional services, temporary Group H requests are best met through your DDSD Regional Office. Requests are made by your case manager.
Long Term Group H Approved Through Group H Review Committee
Long term Group H requests are also made by your case manager and sent to your DDSD Regional Office.
The Group H Review Committee regularly meets to review the long term requests for Group H. The Group H Committee members consist of DDSD staff from a variety of specialty areas such as behavior support, clinical services, community inclusion, and case management.
